Accountants for doctors
Medical earnings are structurally messy: a consultant might hold NHS employment, private practice, medico-legal work and teaching — four tax treatments on one return. Locums add IR35 questions and umbrella-versus-company decisions per assignment. And over it all sits the NHS pension annual allowance, which has surprised more senior doctors with tax charges than any other single rule.
How we handle it
We specialise in exactly this mix: locum returns and company accounts, private practice income properly expensed, the employment/self-employment boundaries kept clean, and annual-allowance positions checked every year with scheme-pays elections handled when charges bite. Medicine pays you six ways; we file it as one coherent position.

UTR
Your Unique Taxpayer Reference — the 10-digit number HMRC issues when you register for Self Assessment. Lifelong, and needed for every return.
Payments on account
Advance payments toward next year's tax, due 31 January and 31 July, triggered once your bill passes £1,000. The first year they apply effectively bills 150% at once.
SA100
The main Self Assessment return form. Supplementary pages bolt on for employment, self-employment, property, capital gains and foreign income.
Balancing payment
The difference between what your payments on account covered and what the year's tax actually was — settled each 31 January.
Statutory accounts
The formal year-end accounts every company must file at Companies House — publicly visible, prepared to FRS standards.
FRS 105 / FRS 102 1A
The two accounting standards small UK companies use: FRS 105 for micro-entities (minimal disclosure), FRS 102 1A for small companies (fuller picture).
CT600
The corporation tax return filed with HMRC alongside a tax computation — due 12 months after year end, though the tax is due at 9 months and a day.
Accounting reference date
Your company's official year-end date, set at incorporation (changeable within limits). Accounts are due 9 months after it.
Accountants for doctors: your questions answered
As a locum, should I use a limited company or an umbrella?
It depends on where your assignments sit under IR35: inside-IR35 NHS work through a company gains little, making umbrellas simpler for those assignments; outside-IR35 work (more common in GP locuming and some private cover) can still reward a company. Many locums run mixed arrangements — legitimate, but worth structuring deliberately.
What is the pension annual allowance problem doctors keep hitting?
Defined-benefit growth in the NHS scheme counts against the annual allowance, and for higher earners the taper can shrink that allowance further — creating tax charges on pension growth you never saw as cash. The scheme-pays route lets the pension settle the charge. We calculate exposure annually so the charge is at least never a surprise.
What expenses can doctors claim?
Against self-employed income: indemnity, GMC fees, exams and courses for existing specialisms, equipment, professional subscriptions, home office for admin. Against employment income the rules are far tighter — professional fees and some subscriptions, claimed through the return. Splitting expenses correctly across your income streams is most of the craft.
Sole trader or limited company for locum work?
It depends on your day rate, session volume and pension priorities — NHS pension access often argues for sole-trader GP work, while non-pensionable and private work can favour a company. We'll model your actual mix rather than give a generic answer.
What expenses can locums claim?
Professional indemnity, GMC/GDC/GPhC registration, royal college fees, courses and CPD, travel between temporary workplaces, and equipment. The travel rules around 'temporary workplace' are nuanced — claiming them correctly is worth real money.
What does the £129 fee include?
Preparation and online filing of your full SA100 return, including the supplementary pages your situation needs. If your return is more complex — rental income, capital gains — the packages above show the fixed price, and we confirm it in writing before starting.
I've missed the deadline — can you still help?
Yes. Late returns are common and fixable: we prepare and file the outstanding return quickly, and where there's a reasonable excuse we help you appeal penalties. The sooner it's filed, the sooner the penalties stop growing.
My company hasn't traded — do I still need to file?
Yes. Even dormant companies must file accounts at Companies House and usually a confirmation statement. Our dormant package (£99) handles it all so you avoid penalties for a company that isn't even trading.
Which accounting standard will my accounts use?
Most small companies file micro-entity accounts under FRS 105 or small-company accounts under FRS 102 Section 1A. We pick the right one for your size and circumstances — it affects what's disclosed publicly at Companies House, and we'll explain the trade-offs.
Is this tax avoidance?
No. We use allowances and reliefs exactly as intended — pensions, ISAs, dividend planning, marriage allowance, timing of income and gains. Nothing we recommend requires disclosure schemes or would trouble an HMRC review. If a plan needs a QC's opinion, it's not our kind of plan.
What's the £100k trap?
Between £100,000 and £125,140 of income you lose the personal allowance at £1 per £2, creating a 60% effective rate — and childcare entitlements can vanish at £100k too. Pension contributions are usually the clean fix. If you're near the line, a review typically pays for itself many times over.
